HMB and sport: what do studies show about recovery and performance?

HMB (beta-hydroxy-beta-methylbutyrate) is a leucine metabolite marketed for recovery and muscle mass. Its relevance depends strongly on the population and exercise context: results are more uncertain in already-trained young athletes than marketing suggests.

What is HMB? HMB is a metabolite of leucine. Calcium HMB and free-acid HMB have been studied. Faster absorption of HMB-FA does not prove better sporting outcomes.

A 2020 meta-analysis in young adults found no significant improvement in lean mass or strength. A 2025 network meta-analysis found null or very uncertain effects across several outcomes in trained athletes.

Muscle-damage markers such as CK or LDH are not the same as performance or competition recovery. One small trial in 23 trained men found faster work-capacity recovery after a highly damaging protocol with single-dose HMB-FA, but other measures did not improve.

When is the question most relevant? The most plausible context is unusual stress or reduced stimulation: starting training, returning after a break, heavy loading, immobilisation or older age. This does not mean HMB prevents injury, treats disease or guarantees recovery. For a well-fed recreational athlete who already trains consistently, added benefit is uncertain.

Studied doses: a reference, not a prescription Trials often use about 3 g HMB daily, sometimes split into three servings over several weeks. The ISSN also discusses about 38 mg/kg/day in some protocols. These are research regimens, not universal doses.
