In brief

The edible heart is food. The long leaves on the stem are the material used in herbal preparations. A leaf extract is therefore not a concentrated artichoke heart, and evidence for one product cannot be transferred to every capsule or tea.

Heart, leaves and fructans

Artichoke is listed by Monash FODMAP among vegetables particularly rich in fructans. These fermentable carbohydrates may increase gas, distension or pain in some people, especially with IBS. Portion size and individual tolerance matter.

Medicinal leaf preparations use dried, powdered or extracted leaves. Their exposure is not equivalent to eating the heart. The extraction solvent, plant-to-extract ratio and label matter.

Traditional use is not the same as clinical proof

EMA says the indication is based on long-standing traditional use. The available clinical studies were small, short or methodologically limited. EMA covers adults and adolescents over 12; persistent symptoms beyond two weeks require medical advice.

What the trials actually tested

A 2003 randomised trial enrolled 247 people with functional dyspepsia and tested a defined commercial leaf extract for six weeks. Symptoms improved more than with placebo. An open 2002 study followed 516 volunteers, but without a placebo group its result is less certain. A ginger-artichoke combination cannot isolate the artichoke effect.

FODMAP tolerance in practice

Fructans are poorly absorbed and fermented in the colon. Inulin belongs to this family. Fermentation may support gut bacteria, but can also cause gas in sensitive people. A food portion and a leaf extract should not be treated as interchangeable.

Biliary disease and other precautions

EMA lists bile-duct obstruction, cholangitis, gallstones and other biliary diseases as contraindications for leaf preparations. Allergy to artichoke or other Asteraceae is another reason to avoid them. Use is not recommended under age 12. During pregnancy or breastfeeding, medicinal-use safety data are insufficient; ask a professional.

People with liver or biliary disease, or those taking regular medicines, should check the product with a doctor or pharmacist. Severe right-upper-abdominal pain, fever, jaundice or repeated vomiting needs medical assessment.

Choosing a form

For food, use a portion you tolerate. For a medicinal preparation, check the plant part, extract type, solvent, extraction ratio and warnings. Monograph quantities describe defined preparations, not a universal supplement dose.

01Is the heart the same as a leaf extract?

No. The heart is food containing fibres and fructans; a leaf extract is a concentrated preparation whose composition depends on extraction.

02Is artichoke low FODMAP?

Monash lists artichoke among vegetables rich in fructans. Tolerance depends on portion and person; avoid prolonged restriction without guidance.

03Does EMA say artichoke cures dyspepsia?

No. It describes traditional use for symptom relief and says clinical evidence is not firm.
