In brief

Coenzyme Q10 (CoQ10) is a fat-soluble molecule found in the body and involved in mitochondrial reactions. Supplements have mainly been studied in heart failure, migraine and some statin-related symptoms, but the results do not support routine use for everyone.

1. What is CoQ10?

CoQ10 mainly occurs as ubiquinone and ubiquinol, two chemical states within the same redox system. Their names alone do not prove that one form is clinically superior for a particular goal. Products also differ in dose, oil-based formulation and manufacturing quality.

2. What does it help with?

In heart failure, a 2024 meta-analysis pooled 33 randomized trials and reported favourable results for several outcomes. It also highlighted study variation and different evidence quality; this does not make CoQ10 a stand-alone treatment. [1]

For migraine, NCCIH describes preliminary findings: some reviews report fewer migraine days or shorter attacks, but the evidence is low quality and the trials are small. [2]

For muscle symptoms associated with statins, results are inconsistent. CoQ10 should not lead anyone to stop or change a statin without medical advice. [3]

3. Ubiquinone or ubiquinol?

Ubiquinone is the oxidized form and ubiquinol the reduced form. Available studies do not show that one is consistently better for every use. Compare the amount per serving, formulation, tolerability and traceability rather than relying on one marketing claim.

EFSA assessments of proposed claims about energy, blood pressure, cholesterol, cognition and endurance did not establish a general basis for presenting CoQ10 as effective for these outcomes in healthy people. [4]

4. Safety and interactions

Reported side effects are usually mild, including digestive upset, headache or insomnia. CoQ10 may interact with warfarin and insulin; NCCIH also notes uncertain compatibility with some cancer treatments. [5]

5. How to assess a product

Check the stated form, amount per serving, full ingredient list and storage instructions. Oil-based formulations are common for fat-soluble products, but this feature alone does not prove a clinical benefit. Start with the actual goal and a review of current medicines.

01Does CoQ10 increase energy?

It participates in cellular reactions related to energy production, but that does not prove that a supplement increases perceived energy in healthy people.

02Can CoQ10 be taken with a statin?

Discuss this with the prescriber. Evidence for muscle symptoms is inconsistent, and CoQ10 is not a reason to stop a statin.

03Are ubiquinol and ubiquinone interchangeable?

They are two chemical forms of CoQ10. Evidence does not establish a general advantage of either form for every use.
