Vitamin B6 supplements: dose, forms and neuropathy risk

Vitamin B6 is found in B-complex products, multivitamins and magnesium products. Water-soluble does not mean risk-free. Adult nutritional needs are about 1.3–1.7 mg/day, while EFSA set a 12 mg/day tolerable upper level in 2023. This is a safety reference, not a target. Add B6 from every supplement and fortified product. Pyridoxine is the common supplemental form; pyridoxal-5-phosphate (P-5-P) is the active coenzyme form. Human evidence does not show that P-5-P is universally superior or risk-free. The practical priority is total milligrams, duration and stacking. Possible toxicity signs include tingling, burning, numbness and balance problems, usually in the hands and feet. If they appear, stop self-medication, keep the packages and seek medical advice; other causes such as B12 deficiency, diabetes, alcohol, thyroid disease or nerve disorders must also be considered. Isoniazid, cycloserine and penicillamine can affect B6 metabolism; levodopa without carbidopa is also relevant, while high pyridoxine doses can affect phenytoin and phenobarbital levels. Pregnancy, breastfeeding, childhood, liver or kidney disease, confirmed deficiency and neurological symptoms require professional advice. A simple method is to state the reason, total all sources, compare with nutritional needs and the EFSA ceiling, and reassess the need. “Active form” does not mean “more effective”; “natural” and “water-soluble” do not guarantee safety.
