Vitamin B9: folates, folic acid and precautions

In brief Vitamin B9 is a family of compounds. Food folates occur naturally in foods; folic acid is the synthetic form used in some fortified foods and supplements; 5-MTHF is another supplement form. They should not be treated as interchangeable. [1][2]
1. Folates, folic acid and 5-MTHF Food folates are found in leafy greens, pulses, some fruit, nuts and liver. Folic acid is a stable synthetic form used in fortified foods and most supplements. 5-MTHF, also called methylfolate, is also sold in supplements. Its presence does not prove general clinical superiority. The prevention evidence for neural tube defects concerns folic acid, not an automatic equivalence between all forms. [1][2]
2. Foods that provide B9 A food-first approach means varying sources instead of looking for a miracle food. Leafy greens, pulses, some fruit, nuts and liver contribute to intake. Amounts vary with food, portion and preparation, so food tables are reference points rather than promises of exact intake. [1][2]
3. European reference values EFSA gives a population reference intake of 330 mcg DFE per day for adults, an adequate intake of 600 mcg DFE during pregnancy and a reference of 500 mcg DFE during breastfeeding. These are population values, not personal prescriptions. [3]
4. Pregnancy planning The first weeks of embryonic development may come before pregnancy is discovered. ANSES and NIH state that folic acid before conception and early in pregnancy reduces the risk of neural tube defects. Timing and dose should be discussed with a professional, especially with a pregnancy plan or an existing pregnancy. [1][2]
5. How to read a supplement label Record the exact form, the amount per unit and every other source: prenatal product, multivitamin, fortified food or medicine. The common error is stacking several labels. EFSA’s adult upper level of 1,000 mcg per day concerns the combined intake of folic acid and authorised 5-MTHF salts. It is not a target, and a prescribed higher dose must not be changed without the prescriber. [4]
6. Deficiency, B12 and interactions A folate deficiency can contribute to megaloblastic anaemia, but fatigue alone does not establish deficiency. EFSA identified progression of neurological signs in people deficient in vitamin B12 as the critical effect for the upper level. High-dose supplementation therefore should not hide a problem requiring diagnosis. Report supplements when taking regular medicines or several products. [2][4][5]
7. A cautious method 1. Define the question: food, test, suspected deficiency, pregnancy or general prevention. 2. List all products and fortified foods. 3. Check form and unit: folate, folic acid, 5-MTHF, mcg or mg. 4. Add synthetic forms and compare them with the upper level and any prescription. 5. Ask a professional when pregnant, planning pregnancy, taking treatment or considering a high dose.

01Are food folates the same as folic acid? They belong to the same vitamin family but are not exactly the same forms. [1][2]
